Complete this form if you are funding a Future Scholar 529 College Savings Plan Account with contributions from a Qualified Tuition Program account, a Coverdell education savings account, a qualified U.S. savings bond or a Uniform Gifts to Minors Act/Uniform Transfers to Minors Act (UGMA/UTMA) Custodial Account.

This form is to be completed to direct any type of withdrawal from a Future Scholar 529 College Savings Plan Account, or to direct a transfer rollover from an Account.
